GLP-1 Drugs Melt Fat, Not Muscle, Vienna Data Show

For years, doctors worried that GLP-1 drugs might strip muscle along with fat. New data out of Vienna suggest that fear was overblown. Researchers tracked 486 adults at a specialized obesity clinic and found the weight patients shed was overwhelmingly fat, not lean tissue.

The team, from the Medical University of Vienna, used bioelectrical impedance analysis to monitor patients on semaglutide, liraglutide, or tirzepatide between 2022 and 2025. After roughly fourteen months of treatment, patients lost an average of 9.9 percent of their body weight. Fat accounted for about 18 percent of that drop. Muscle mass slipped by only around 5 percent.
More than seven in ten patients held onto their relative skeletal muscle mass, and some actually gained ground. Once researchers adjusted for fat loss, treatment length, age, sex, and starting body mass index, muscle mass stayed essentially stable. That points to a healthy shift in body composition rather than a warning sign.

The study has limits worth flagging. It leaned heavily female, carried no placebo group, and relied on retrospective records rather than a controlled trial. Patients also followed standard exercise advice alongside their medication, which likely helped preserve muscle.

Even with those caveats, the findings offer something clinicians have wanted for a while: real-world reassurance. Structured GLP-1 treatment, paired with basic activity guidance, appears capable of delivering durable weight loss without disproportionate muscle loss during dose tapering and long-term maintenance.
